CAPA Liability Insurance Application Form

This application form is for inclusion into the Master Certificate of Liability Insurance policy, and is intended solely for the Member Clubs in good standing, of

CAPA – CANADIAN ASSOCIATION FOR PHOTOGRAPHIC ART.

The liability policy is for $5,000,000.00

Privacy Breach Extension ($31.00 premimum) is a mandatory purchase.

This is a one size fits all Group/Master Liability Insurance policy and customization for individual clubs/situations is not available in order to keep costs at a minimum.

Cost for each club per year is as follows:

  • Up to 20 Members – $150.00 + $31.00 + $10.00 = $191.00
  • 21- 50 Members – $175.00 + $31.00 + $10.00 = $216.00
  • Over 50 Members – $200.00 + 31.00 + $10.00 = $241.00

Membership cost is based on your club’s current membership. The number of members will need to be updated every year at renewal.

There may be slight cost adjustments depending on which province your club is located in, if there are any provincial tax requirements.
This will be communicated to the clubs once the policies are written.

If your club’s current Liability Insurance is with Co-operators, once your club is covered by this master policy, you have the option to cancel your current policy. To do this, download the Cooperators cancellation form and provide it to your current agent via email or in person.

If your policy is with another company your club will be responsible for arranging cancellation and obtaining any refunds due to you.

 

Payment Options for Liability Insurance:

PayPal:
To pay via PayPal, log in to your PayPal account and send the funds to membership@capacanada.ca. Please indicate that the payment is for General Liability Insurance and include your Club Name.

Cheque Payment:
Make your cheque payable to Canadian Association for Photographic Art and mail it to:

*Ensure that the cheque notes it is for General Liability Insurance and clearly states your Club Name.

E-transfer:
Send your payment via e-transfer from your financial institution to :  capaoffice@capacanada.ca Include your Club Name and General Liability Insurance as the payment reference.

Credit Card:
You can pay by credit card over the phone during regular CAPA office hours. Call Eleanor Campbell at 1-236-586-3938to process your payment.

 

*Please note that your coverage will only take effect once CAPA has received your annual payment.

 

Note regarding D&O Insurance

D&O insurance is only available as an add on policy to the clubs that are ‘REGISTERED Non-Profits’ in their province. minimum of 25 clubs would be needed to sign up for this Option for it to be offered by Cooperators Insurance for $200.00 per club. We may offer this insurance if we meet the requirement of having 25 clubs that qualify.
